Is cycling about on-the-cushion AND/OR off-the-cushion experience?

About how long can one expect a cycle to take, with various degrees of practice?

Does a cycle start at Stage 1 or Stage 4? And do they always go to Fruition?

If stream enterers cycle (as the passage seems to imply), wouldn't they cease being stream enterers after one cycle?
